What is Remote Patient Monitoring?

Remote Physiologic Monitoring, also known as Remote Patient Monitoring, enables a practise to track a patient’s vital signs without them having to come into the clinic or sending them by email or patient portal. Remote Patient Monitoring does not include manual vitals measurement submission. Real-time vitals monitoring, such as blood pressure, allows healthcare doctors to view daily measurements with remote patient monitoring devices and change medications based on real data rather than just the readings taken during an office visit.

How it Works for Providers?

Sign Up

You can register by entering the required information. This does not necessitate the use of a credit card. We will send you a couple test devices for testing reasons once the enrollment procedure is done.

Enroll

At no additional charge, Secure Health Services will enroll your patients as a complementary service. After validating the patients' eligibility for Remote Physiological Monitoring, we will send the gadgets straight to them.

Patient Data

You'll be looking over patient data that was sent via app. For the completion of this process, we will notify you once a month. You must comply with all Medicare standards. The patient data will be sent to the c-lynx portal in real time.

Billing
Your biller should submit CPT 99453, 99457, 99458, and 99454 to Medicare. After Medicare payment, we debit your account for CPT 99454, minus co-pay, to cover hardware, cellular services, hosting, and support.

CPT Codes

The Current Procedural Terminology (CPT) coding system is a set of codes and descriptions used by health care providers to identify the services they provide. CPT codes are used for billing, reimbursement, and quality reporting.

CPT 99453 (Setup)

Initial setup, remote monitoring of physiologic parameter(s) (e.g., weight, blood pressure, pulse oximetry, respiratory flow rate), and patient education on equipment use.

CPT 99454 (Device & Monitor)

Initial device(s) supply with daily recording(s) or programmed alert(s) transmission, every 30 days; remote monitoring of physiologic parameter(s) (e.g., weight, blood pressure, pulse oximetry, respiratory flow rate), initial; device(s) supply with daily recording(s) or programmed alert(s) transmission

CPT 99457 (Patient Engagement)

Remote physiologic monitoring treatment management services that require 20 minutes or more of clinical staff/physician/other qualified healthcare professional time in a calendar month and interactive conversation with the patient/caregiver.
